Situation

You write 'never run X' in CLAUDE.md and expect it to be blocked.

Symptom

The agent sometimes runs X anyway.

Cause

Claude treats CLAUDE.md files as context, not enforced configuration.

Fix

To block an action regardless of the model's decision, use a PreToolUse hook or permission rules.

Verify

Try the forbidden action in a test session. It must be blocked by the hook, not by the model's choice.
